Determining Your Eligibility
Upon your first visit to our office we will verify your coverage in our office and answer any questions you may have. When you call to make your appointment, please have your insurance card/information ready so we may gather your insurance information before your visit. As a no-cost service to you, we strive to have that information when you appear for your first visit so you will know exactly what to expect for your first day in advance.
If you have any immediate questions regarding your insurance provider’s coverage, please contact the number on the back of your health plan membership ID card, or refer to your benefit plan document.
